- Enter RMO/House Surgeon details including name of Homeopathic Medical College & Hospital, whether recognised by NCH/MCI (Yes/No), period (From–To) and total.
- 11. Total marks claimed:
- Fill the ‘Total Marks Claimed by Candidate’ grid for BHMS/MBBS, MD/MS, Journal Publications (Max 04) and Teaching Experience (02 marks per year, Max 10).
- 12. Performa-II declaration:
- Sign and date the declaration at the end of Performa‑II confirming that the information provided is true and correct.
Selection Process
- 1. Evaluation components:
- Performa‑II captures claimable marks for BHMS/MBBS and MD/MS academic records, journal publications and contract teaching experience as per the advertisement.
- 2. Journals weightage:
- Journal publications carry marks with a maximum of 04 marks (as printed in Performa‑II).
- 3. Teaching experience weightage:
- Contract teaching experience is counted at 02 marks per year with a maximum of 10 marks (as printed in Performa‑II).
Reservation Details
- 1. Certificates for reservation:
- SC/ST/OBC/EBC/EWS/PwBD/Ex-servicemen must produce valid certificates in the prescribed format at verification.
- 2. Non-creamy layer:
- OBC/EBC applicants must bring a valid non-creamy layer certificate.
- 3. Domicile for reservation:
- State reservation is admissible only to Bihar domiciled candidates on production of Bihar domicile/residence certificate.

- NTET certificate/score card issued by the concerned national council/commission, where applicable.
- 10. Teacher profile/code:
- Teacher Profile/Teacher Code issued by the concerned Council/Commission, if any.
- 11. Caste certificate:
- SC/ST/OBC/EBC certificate in prescribed format; OBC/EBC must be non-creamy layer.
- 12. EWS certificate:
- Economically Weaker Sections certificate in prescribed format, if claimed.
- 13. PwBD certificate:
- Disability certificate issued by the competent medical board, if claimed.
- 14. Ex-servicemen:
- Ex-servicemen certificate/discharge book, if claimed.
- 15. NOC/forwarding letter:
- No Objection Certificate or forwarded application for in-service applicants.
- 16. Domicile:
- Bihar domicile/residence certificate wherever required for reservation.
- 17. Photo ID:
- Valid photo identity proof such as Aadhaar/Voter ID/Driving Licence/Passport.
- 18. Photographs & copies:
- Two recent passport-size photographs and one set of self-attested photocopies of all documents.
- 19. Name change proof:
- Affidavit/Gazette notification or supporting proof in case of name/surname mismatch.
- 20. NCISM Teacher Profile (Ayurveda):
- Copy of Teacher Profile from NCISM to be attached where applicable (form provides tick for Attached: Yes/No).
- 21. NCISM Teacher Code email (Ayurveda):
- Candidate’s copy of email regarding generation of Teacher Code by NCISM to be attached where applicable.
- 22. NTET score card enclosure (Homoeopathy):
- Attach NTET score card/certificate if applicable (form provides field for ‘Score Card Attached: Yes/No’).
- 23. Performa-II:
- Duly filled and signed Performa‑II as provided in this document.
Important Instructions
- 1. Be punctual:
- Report to the venue before the start time of your allotted shift; late entry may not be permitted.
- 2. Originals mandatory:
- Carry all originals with one set of self-attested photocopies; non-production may result in cancellation.
- 3. Forms to be filled:
- Complete the verification forms/undertakings (Annexure-I/II) as required and submit along with documents.
- 4. Venue:
