- Physical document verification at BPSC office in two sessions each day (10:00 AM and 2:00 PM).
- 2. Allotment to sessions:
- Roll/Application numbers allotted to first/second session are printed in the notice against each advertisement.
- 3. Reporting:
- Report with the printed Document Verification Letter and required documents; without it, entry will be denied.
- 4. Final chance DV:
- Absent candidates of Advt. 68/2025–78/2025 must appear for document verification between 15.07.2026 and 17.07.2026 at the Commission office.
- 5. Corrigendum (Advt. 77/2025):
- Eight candidates’ Roll/Application numbers in the DV list were misprinted (07025.. instead of 07725..); DV date for them remains 29.05.2026 as per programme published on 06.05.2026.
- 6. Form submission:
- Submit the filled and signed Form‑I (Prapatra‑I) at the DV desk along with originals and photocopies of certificates as indicated in the form.
- 7. Form‑II contents:
- Performa‑II captures BHMS/MBBS and MD/MS academic marks, teaching experience in Govt./Non‑private medical colleges within Bihar (contract), journal publications and RMO/House Surgeon details, and the total marks claimed.
- 8. Scoring — teaching experience:
- 02 marks per year for eligible contract teaching experience (maximum 10 marks) as printed on Form‑II.
- 9. Scoring — publications:
- Journal publications carry up to 04 marks; indexing agency must be as per Advertisement Clause 5(i) Note (b).

- Form‑II contains a mandatory declaration to be signed by the candidate; false information can lead to cancellation and legal/panel action.
