- 1. Process step:
- First-phase Document Verification will be conducted as per the advertised schedule for each advertisement number.
- 2. Shifts:
- Two shifts each day: 10:00 AM (first shift) and 2:00 PM (second shift).
- 3. DV Letter:
- Download the Documents Verification Letter from the candidate portal (My Account → View → Documents → Download).
- 4. Attendance basis:
- Reporting is strictly as per the Roll No./Application No. lists published for each shift.
- 5. Venue:
- BPSC Office, 15, Nehru Path (Bailey Road), Patna – 800001.
- 6. Supplementary DV window:
- An additional document verification window will be held from 15.07.2026 to 17.07.2026 at the Commission office, Patna for absentees of the first phase.
- 7. Prior DV letter valid:
- The verification letter issued earlier will be accepted for the supplementary DV.
- 8. Follow earlier instructions:
- Candidates must follow the instructions published with the verification programme on 06.05.2026 on the BPSC website.
- 9. Corrigendum – Advt. 77/2025:
- Eight candidates’ Roll/Application numbers were corrected; those candidates must report for DV on 29.05.2026 at BPSC, Patna as per the programme dated 06.05.2026.
- 10. Form-I proforma:
- This Form‑I is to be filled by candidates in blue/black ink for the document verification process under Advt. 68–78/2025.
- 11. Form-II proforma (this notice):
- Candidates must fill Proforma‑II providing BHMS/MBBS, MD/MS, contract teaching experience in Bihar, journal publications and RMO/House Surgeon details, and sign the declaration.
- 12. Weightage components:
- Proforma‑II records marks components: BHMS/MBBS, MD/MS, Journal Publications (maximum 04 marks) and Teaching Experience on contract @ 02 marks per year (maximum 10 marks), as per the advertisement.

- 2. Application printouts:
- Printouts of the online application (Part–1 and Part–2).
- 3. Originals and photocopies:
- All originals with a set of self–attested photocopies of every document submitted in the application.
- 4. DOB proof:
- Matriculation/Equivalent certificate as proof of date of birth.
- 5. Educational certificates:
- Certificates and mark–sheets of the required qualifications (as applicable for the concerned advertisement).
- 6. Internship:
- Internship completion certificate (where applicable).
- 7. Registration:
- Registration certificate with the Homoeopathic/State Medical Council (as applicable).
- 8. Category certificates:
- SC/ST/BC/EBC/EWS certificates and Non–Creamy Layer certificate where applicable, issued by the competent authority of Bihar.
- 9. PwBD certificate:
- Valid disability certificate, issued by the competent Medical Board (expired/renewal–due certificates will not be accepted).
- 10. Experience/NOC:
- Experience certificates (if claimed) and No Objection Certificate for serving Government employees.
- 11. Identity proof:
- Government photo ID (e.g., Aadhaar/PAN/Voter ID/Driving Licence).
- 12. Photographs:
- Recent passport–size photographs as required.
- 13. Teacher Code (if any):
- Teacher Code issued by the National Commission for Homoeopathy, if available.
- 14. DV letter validity:
- The DV letter issued earlier will be valid for the supplementary verification.
- 15. Form-I proforma:
- Form‑I (Document Verification proforma) duly filled and signed in blue/black ink.
- 16. Photograph on proforma:
- Affix a recent passport‑size photograph on Form‑I so that the attestation overlaps both the form and the photograph.
- 17. NTET documents:
- NTET score card/eligibility certificate and related documents, if applicable.
- 18. Teacher Code supporting:
- Copy of Teacher Profile from NCISM and candidate copy of email regarding generation of Teacher Code by NCISM, if applicable.

- 22. Salary slip (if claimed):
- Salary slips supporting contract teaching experience as referenced in Proforma‑II.
- 23. RMO/House Surgeonship proof:
- Certificate/record of RMO/House Surgeonship from a Homoeopathic Medical College & Hospital recognised by NCH/MCI, indicating the period.
Important Instructions
- 1. Report on time:
- Candidates must report on the scheduled date and shift for their advertisement; reach before the reporting time mentioned (10:00 AM or 2:00 PM).
- 2. DV letter not by post:
- The Documents Verification Letter will not be sent by post; download it from the candidate portal.
