- On Proforma‑II, write the Advertisement No., concerned Department and your Application/Roll Number.
- 7. Academic sections:
- Fill BUMS (all years) total and percentage, and MD/MS (Unani) subject, session, passing date and college as per the columns.
- 8. Experience section:
- Enter details of contract teaching experience in Unani/Tibbi Medical Colleges within Bihar (post, department, college, contractual yes/no, state, Govt./Private, from‑to, total experience).
- 9. Publications section:
- State the number of journals published till 25-08-2025 in figures and in words, and provide title, type, main author status, journal name, indexing agency, UGC CARE/SCI/Scopus/PubMed/Web of Science serial/year, ISSN and date of publication.
- 10. Declaration on Proforma‑II:
- Sign the declaration on Proforma‑II confirming that the information provided is true and correct.
Selection Process
- 1. BUMS academic marks:
- Candidates claim BUMS academic marks as per Advertisement Clause 5(i)(क), entered on Proforma‑II.
- 2. MD/MS (Unani) marks:
- Academic marks as per Advertisement Clause 5(i)(ख) carry 10 marks, to be recorded on the form.
- 3. Contract teaching experience:
- Experience in Govt./Non‑Private Unani/Tibbi Medical Colleges within Bihar on contract basis earns 2 marks per year (maximum 10 marks) as per Clause 5(i)(ग) and Clause 5(ii) Note (a).
- 4. Journal publications:
- Publication marks are capped at 4; only journals published up to 25-08-2025 are to be entered with indexing/ISSN details.
- 5. Candidate declaration:
- A signed declaration on Proforma‑II affirms the correctness of information; incorrect details may lead to legal/panel action and cancellation.

- Experience Certificate number and issue date for each claimed contract teaching post (as per Proforma‑II columns).
- 2. Appointment letter:
- Appointment letter and its issue date for the contract teaching experience posts.
- 3. Salary slip:
- Salary slip(s) supporting the claimed contract teaching experience (Yes/No column on the form).
- 4. Publication certification:
- HOD/Guide/Prof. In‑charge certification for each listed publication, along with indexing/ISSN details as printed on the form.

- 1. Reporting:
- Report on the scheduled date and shift mentioned for your advertisement number with DV letter and documents.
- 2. Originals and copies:
- Carry original certificates with self-attested photocopies for verification.
- 3. Cut-off for claims:
