- Appendix-I – Certificate regarding physical limitation in an examinee to write / Certificate for person with specified disability (less than 40%) having difficulty in writing.
- 2. Authority & signatures:
- Signed by specialist(s) of the relevant disability stream (e.g., Orthopedic/PMR, Clinical/Rehabilitation Psychologist/Psychiatrist/Special Educator; Neurologist/Occupational therapist if available) and by the Chief Medical Officer/Civil Surgeon/Chief District Medical Officer (Chairperson).
- 3. Institution seal:
- Must include the name of the Government Hospital/Health Care Centre with official seal.
- 4. Fields to be filled:
- Candidate’s name, S/o or D/o, address (Village/District/State), nature and percentage of disability/condition, statement of writing limitation, list of essential aids/assistive devices, place and date.

- Certificate is issued only for the purpose of appearing in written examinations conducted by recruitment agencies or academic institutions.
- 2. Scribe confirmation:
- Wording affirms that the candidate requires support of a scribe for writing the examination.
- 3. Assistive aids note:
- Names of aids/assistive devices (such as prosthetics, orthotics, hearing aids) essential for the examination must be specified.
- 4. Validity period:
- Valid for a maximum of six months or less as certified by the medical authority.
