Name of the Reporting Officer, Designation, Mobile Number and Email ID.
4. Declaration:
Candidate declares they have intimated their employer in writing about submitting the online application to CSL.
5. Consent & consequence:
Candidate has no objection to CSL contacting the employer; if any objection is received from the employer, the candidate will have no claim for selection.
6. Signature, place & date:
Form should carry the candidate’s signature, place and date.
